I do hereby acknowledge that I have, of my own free will, voluntarily applied to participate in circus acrobatics practice, instruction and training, including activities on the flying trapeze and other circus skills conducted by Listo Trapeze Company.

I am aware that circus acrobatics, including acrobatics on the flying trapeze, trampoline, Spanish web, single trapeze, double trapeze, teeterboard, and other catapult device and on tight wire or other such balancing devices are dangerous and hazardous activates I do so at my own risk. I am voluntarily participating in these activities with the knowledge of the danger involved and here by agree to accept any and all risks of injury and death. I am aware that as a condition on admission to instruction in the activities, that Listo Trapeze Co, its directors, instructors, members, servants, or agents are absolved from all liability howsoever rising from injury or damage howsoever caused (whether fatal or otherwise) arising from my participation in the above activities or in any way whatsoever due to the neglect act, breach of duty, default and/or omission on the part of Listo Trapeze Co, it's directors, instructors, members and/or servants.

In consideration of being permitted to undertake the above activities I here by agree that I, my heirs, assigns and legal representatives will not make any claim against list trapeze co and/or it's directors, members, servants or agents for any injury or damage arising from any negligence of any of the said parties or howsoever caused.

I here by irrevocably grant Listo Trapeze Co permission to use and publish photographs of me participating in activities conducted by the Company and which may be taken by directors, instructors, servants, agents and/or contractors of the company. This authority shall  include the instructors , servants, members agents and/or contractors of the Company. This authority shall include the right to publish or use such photographs in publication, in part or in whole, for advertising or marketing purposes without any further authority of inspection by me.
